Louisiana Living Will is a complete Freebie!  It does not require that your signature be notarized.  Anyone over the age of 18 can be a witness, except people who will receive anything from your estate should you die.  Print out a copy of the web-page, make photocopies, then sign (original signatures) as many photocopies as you think you will need (a photocopy of an already signed Living Will is no good).  You will need one for your doctor's office, an extra one for the doctor to take with him to the hospital, one for your wallet/purse, one for the glove-compartment of each vehicle, one or two for each child (or other relative) who may be contacted if you wind up in a hospital, etc.
